Executive Summary
Our community includes a very special population of individuals who, despite their significant health needs, are often overlooked in healthcare; these are individuals with intellectual and developmental disabilities (I/DD), including autism, cerebral palsy, and Down syndrome. I/DD conditions are ubiquitous across all groups, ages, and genders. This population experiences higher inequities leading to poorer health, higher rates of chronic diseases, mental health concerns, and higher rates of preventable mortality. Nonetheless, access to quality healthcare is grossly inadequate.
Achievble Health is a first-of-its-kind community health center with services tailored to meet the whole-person health needs of individuals with I/DD. Our aim is to create a paradigm shift so that this community is no longer on the fringes, but rather has access to healthcare, are partners in their care, and have a sense of belonging. Our model was born out of the need to break down barriers for those with I/DD, but we soon understood that all underserved communities had similar needs and we expanded our focus. As such, our mission is to provide high quality, integrated healthcare to individuals with I/DD, their families, and other vulnerable populations. Geographically we serve a large area of Los Angeles County with some of the lowest health outcomes.
We provide comprehensive primary and behavioral healthcare services to over 2,300 individuals. Our board's majority is our consumers, empowering them through their voice. Our staff represents the diversity and lived experiences of our patients. Together we partner to improve the health outcomes of our community.
